CONSENT TO DISSEMINATE PERSONAL DATA WITH THE SOCIAL SECURITY OFFICE
The Social Security Office (SSO) would like to update the insured person's database under Section 33 in order to keep them informed of the news, benefits and timely relief from the distressing situation caused by COVID-19.
The Office of Human Resources Management (OHRM), as a “Personal Data Controller”, sees that the SSO's aforementioned actions are compliant with the Personal Data Protection Act B.E. 2562. OHRM will provide your current address and contact number to SSO in order to maintain the personnel’s basic rights and health benefits.
If you deny providing such information, please notify OHRM via email (This email address is being protected from spambots. You need JavaScript enabled to view it.) by Friday, August 6, 2021. After the given date, you hereby certify that you permit OHRM to share your information with SSO.

ความยินยอมให้ข้อมูลส่วนบุคคลแก่สำนักงานประกันสังคม
ด้วยสำนักงานประกันสังคมมีความประสงค์ในการปรับปรุงฐานข้อมูลของผู้ประกันตนมาตรา 33 ให้เป็นปัจจุบัน เพื่อใช้ในการประชาสัมพันธ์ข่าวสาร สิทธิประโยชน์ และการติดตามให้ความช่วยเหลือเยียวยาได้ทันท่วงทีกับสภาวการณ์ความเดือดร้อนจากโควิด-19
สำนักงานบริหารทรัพยากรบุคคลในฐานะ “ผู้ควบคุมข้อมูลส่วนบุคคล” เห็นว่าการดำเนินการดังกล่าวของสำนักงานประกันสังคมเป็นไปตามพระราชบัญญัติคุ้มครองข้อมูลส่วนบุคคล พ.ศ. 2562 สำนักงานฯ จะดำเนินการส่งข้อมูลที่อยู่ปัจจุบันและหมายเลขโทรศัพท์ของท่านให้แก่สำนักงานประกันสังคมเพื่อรักษาสิทธิขั้นพื้นฐานและสิทธิประโยชน์ด้านการสาธารณสุขของบุคลากรต่อไป
หากท่านปฏิเสธการให้ข้อมูลดังกล่าว ขอให้แจ้งยกเลิกความยินยอมเป็นลายลักษณ์อักษรผ่านทางอีเมล (This email address is being protected from spambots. You need JavaScript enabled to view it.) ภายในวันศุกร์ที่ 6 สิงหาคม 2564 กรณีเลยกำหนดวันดังกล่าวแล้ว สำนักงานฯ ถือว่าท่านยินยอมให้นำส่งข้อมูลดังกล่าวแก่สำนักงานประกันสังคม
